- Fill in your information, events, and check all that apply
- Attach any evidence you have
- Sign and date under UCC 1-308 (no notary needed)
🌿 Step 2: Enroll as a Beneficiary
- Fill out the Beneficiary Enrollment Form
- Declare your standing in truth and conscience
- Submit together with your claim
